Asthma Medications at School
All Health Forms are required to be updated by a health care provider every year and submitted in August.
All students who require asthma medication at school (both in RN office and Self-Cary) must submit the D105 Asthma Action Plan form that has been completed by their health providers and signed by both providers and a guardian.
Medications will only be accepted at school if the Action Plan (including medication, doses, and instructions) is signed by the Health Care Provider/Guardian and the exact medications are presented to building nurses at the same time.
Guardians can initial the form to give permission for students to self-carry, but the health office will still require a completed Action Plan with prescribed medication order.
