Here, you'll find required health documentation forms and information on our wide range of services. We’re here to help your child succeed and to make sure our school community remains strong and healthy.
Please return completed physical exams, immunization records, dental and vision exams to the school’s “After Hours” drop box. These are located outside each school's main entrance. The main offices will no longer accept medical or health paperwork.
MEDICATION AND DOCUMENTATION PROCEDURES

Medication and medication forms can only be given to a D105 school nurse and checked for compliance before being accepted at school. No medication or medication forms will be accepted unless all paperwork is complete, and it is turned in directly to a school nurse. This includes Emergency Action Plans signed by the HealthCare Provider. D105 will provide several Student Health Drop Off Days throughout the summer for parents to bring in paperwork and medications. These can also be turned in during normal school nursing hours (8:30-3pm) and anytime by appointment.
D105 HEALTH DOCUMENTATION REQUIREMENTS
Illinois law requires that all children entering Pre-Kindergarten, Kindergarten, 2nd grade, 6th grade, and students new to Illinois schools submit required state health forms before attending. Please see the requirements below if your child falls into one of those categories.
Completed Forms are due by the first day of the 2025-2026 school year - August 20th, 2025.
Physical examination and proof of immunizations: Required for all students on or before the first day of school (can be dated as early as one year prior to first day of school). Students cannot attend school without these.
Vision exam: Required of all students and due by October 15th of the current school year (can be dated as early as one year prior to the first day of the current school year).
Dental exam: Required of all Kindergarten students and due by May 15th of the current school year. Can be done between November 11th of the prior school year and May 15th of the current school year.
Physical exam/required immunizations: Required of all students on or before the first day of school (can be dated as early as one year prior to the first day of the current school year). Students cannot attend school without these.***Sports Physicals will not be accepted***
Dental exam: Required of all students and due by May 15th of the current school year. Can be done between November 11th of the prior school year and May 15th of the current school year.
Physical exam/required immunizations: Required for all transfers and due at time of registration. Out-of-State transfers need a new Illinois physical exam.
Dental exam: required in grades K, 2, 6.
Vision exam: required for K or any child entering an Illinois school for the first time at any grade
